The document is on a table next to a whiteboard which contains a drawing of flats.
The content of the acquired note contains a hint to determine the code required to open a safe.

you could obtain it soon after you get out of the building where a group of infected was wandering.
You’ll find the document near a corpse.
They’ll allow you to get inside the blue Westport Optix building.

Just after entering the building, turn right and use the blue canopy exit shown in the picture.
The coin lies at the very end of the blue canopy.
Not long after using the ladder, you will reach office 808 of a ruined building.
The document lies on one of the desks, next to two display monitors and a computer keyboard.
After defeating all enemies, Abby will have to use the ladder shown in the picture.
You will reach the location where the altar is located.
Look around the area to find the workbench.

You’ll find it when you get out of the elevator.
